​Eligibilty Guidelines

1.  Applicant must reside in FL, NJ, or MD.​

 

 2.  Applicant must have a life-threatening diagnosis from a doctor, or healthcare provider, and be under medical supervision and care for the diagnosed illness.

 

3.  Travel is limited to the state in which the applicant resides, and within our community partnership destinations and locations.​

 

4.   Applicant must be able to travel within 90 days of approval. The IWF Directors will decide the date and destination.

 

5. If the wish does not occur within 90 days of application, the applicant will be required to submit a new request and application.

 

6. Applicants must have a primary adult caregiver available to travel on trip. Additionally, only the applicant's immediate children within the home are permitted on the requested trip, if applicable. A maximum of four family members, including the applicant's children, will be considered.

 

7.  Applicant understands that IWF reserves the right to accept or deny the request after review by the IWF Directors.​

 

8.  To be considered for a wish, all forms must be completed in their entirety. Any forms submitted with incomplete or missing information will delay the review process.  

 

​9.  All wish applications are reviewed monthly by the IWF Directors.

10. Application for a wish trip does not guarantee eligibility or approval.
